Here you'll find the required forms for ordering your medications

Is a prescription required?
Yes. The pharmacy requires a valid prescription for every
medication order. The only case where a prescription is not
required is if the medication you are ordering is an over-the-counter
item. We do not originate any prescriptions nor do we have
prescribing doctors on our staff.
If I am an existing customer,
do I need to complete the order forms again?
No. Once we have your order forms completed, we update all
of the information on the forms over the phone during the
refill process. This includes the allergy and customer health
profile.
How do I re-order my medications?
Refills can be completed over the phone by dialing 1-800-794-8552
or by sending a fax request to 1-866-868-2303. Please state
the medications you would like to refill, the quantity requested
and a method of payment. How
